A rheumatologist deals with the prevention, diagnosis, and treatment of rheumatic diseases such as connective tissue diseases, osteoarthritis, seronegative spondyloarthropathies, bone and cartilage diseases including osteoporosis. Rheumatic diseases, mostly of unknown aetiology, are characterised by a chronic inflammatory process based on various autoimmune reactions. It is important to remember that rheumatic diseases are systemic diseases, meaning the inflammatory process can affect various organs and systems of our body, causing their function impairment and ultimately their failure. Rheumatic diseases cause limitations in daily activity, a decrease in quality of life, and if untreated, lead to disability and increased mortality. Therefore, early diagnosis and appropriate treatment of these diseases are very important.
The rheumatologist's responsibilities include the prevention, diagnosis, and treatment of:
a) systemic connective tissue diseases
- rheumatoid arthritis and juvenile idiopathic arthritis
- systemic lupus erythematosus
- antiphospholipid syndrome
- systemic sclerosis
- polymyositis and dermatomyositis
- necrotising vasculitis, e.g. eosinophilic granulomatosis with polyangiitis (Churg-Strauss syndrome), granulomatosis with polyangiitis (Wegener's granulomatosis), giant cell arteritis including temporal arteritis, Takayasu's arteritis, polyarteritis nodosa
- Sjögren's syndrome
- mixed connective tissue disease, undifferentiated connective tissue disease, overlap syndromes
- polymyalgia rheumatica
- erythema nodosum
- Still's disease
b) osteoarthritis
c) seronegative spondyloarthropathies
- ankylosing spondylitis
- reactive arthritis
- psoriatic arthritis
- arthritis associated with inflammatory bowel diseases
d) Other conditions, such as: gout, osteoporosis, cartilage inflammation
When should you consider visiting a rheumatologist?
- in case of suspected osteoporosis
- suspected gout
- recurring pain and swelling in one or multiple joints accompanied by stiffness and/or limited mobility; often the symptoms are most severe in the morning, right after waking up; the affected joint(s) are warmer and/or red
- despite being young, you suffer from back pain (cervical and/or thoracic and/or lumbar), lumbar pain radiates to one or both buttocks, groin, or thighs; additionally, an ophthalmologist has diagnosed you with uveitis, you or close relatives have been diagnosed with psoriasis or inflammatory bowel disease; you have Achilles tendonitis (pain, redness, palpable thickening near the heel)
- you suffer from dry eyes, a feeling of no tears, and a sensation of sand under the eyes, additionally you have a dry mouth, a feeling of no saliva, you cannot eat a meal without drinking, you have recurring dental caries, swallowing problems
- you complain of muscle pain in the limbs with accompanying weakness; for some time you have had trouble lifting your arms, combing/drying your hair, you cannot squat, you have difficulty climbing stairs, you have neck pain and stiffness
- you have noticed changes in your facial features: narrowing of the mouth, nose; the skin on the face, torso, and limbs thickens and smooths out, the skin on the hands and feet feels tight, you cannot make a fist, you have difficulty swallowing, frequent breathlessness, reduced exercise tolerance, a dry cough, fingertip ulcers
- you have recurring, hard-to-heal erosions/ulcers in the mouth, skin changes in the form of a rash, erythema, excessive hair loss, sensitivity to sunlight
- if the above-mentioned symptoms are accompanied by, for example: low-grade fever, weight loss, chronic fatigue, skin changes such as erythema, rash, excessive hair loss, chronic sinusitis, asthma, headaches
- if the above-mentioned symptoms are accompanied by abnormalities in laboratory tests, for example: abnormalities in blood count (thrombocytopenia, anaemia, leukopenia), elevated inflammatory markers (ESR, CRP), increased creatinine level, reduced eGFR, elevated ALT, AST, or CPK, presence of protein or blood in urine, presence of rheumatoid factor (RF) and/or anti-citrullinated protein antibodies (ACPAs), presence of HLA-B27 antigen, presence of antibodies: antiphospholipid, pANCA, cANCA, antinuclear antibodies (ANA)

- if you have them, bring your recent laboratory test results, such as complete blood count with differential, ESR, CRP, urinalysis, ALT, AST, creatinine GFR, rheumatoid factor, imaging test results such as X-rays of the hands, feet, chest, including film or CD with the examination, bone densitometry results, abdominal ultrasound report or joint ultrasound examination
- if you have been hospitalised in a rheumatology department or elsewhere where a rheumatic disease was suspected, bring your hospital discharge summary
