Terms of Use for Medical Vouchers of OrtoGo Medical Centre
1. General provisions
1.1. These terms and conditions define the rules for using medical vouchers issued by OrtoGo Medical Centre Limited Liability Company, based in Warsaw, at ul. Wiatraczna 25/U2, 04-384 Warszawa, registered in the Register of Entrepreneurs of the National Court Register maintained by the District Court for the capital city of Warsaw in Warsaw, XIV Commercial Division of the National Court Register under the number KRS 0000718744, NIP 1132963150, REGON 369479904, hereinafter referred to as the "Company".
1.2. The Company is the owner of the medical vouchers (hereinafter: "Vouchers"), which entitle the holder to discounts on medical consultations at facilities operated under the OpenMed brand.
1.3. Vouchers provide a 10% or 20% discount on the cost of medical consultations, excluding: blood collection, laboratory tests, ultrasound examinations, medications, and treatments.
2. Issuance and validity of Vouchers
2.1. Vouchers are issued by the Company based on its own discretionary decision.
2.2. The validity period of the voucher is specified on the voucher and cannot be extended.
3. Use of Vouchers
3.1. Vouchers can only be used at medical facilities operated by the Company (hereinafter: "Facilities").
3.2. To use the Voucher, you must inform about its possession when registering for an appointment, and the paper version of the Voucher must be presented at the Facility's reception before paying for the medical consultation.
3.3. Vouchers are not exchangeable for cash or other benefits.
3.4. Vouchers are single-use and can only be used for one service in the form of a medical consultation.
3.5. Vouchers cannot be combined with other vouchers, discount coupons, or promotional offers.
4. Liability and complaints
4.1. The Company is not responsible for the loss, theft, or destruction of the Voucher after its issuance.
4.2. Any complaints regarding the use of vouchers must be submitted to the Company within 3 days of using the voucher by post to the address of the Company provided in point 1.1.
4.3. Complaints will be reviewed by the Company within 30 days of their submission.
5. Final provisions
5.1. The Company reserves the right to amend these terms and conditions at any time. The Company will inform about any changes to the terms and conditions on its website www.ortogo.pl.
5.2. In matters not regulated by these terms and conditions, the applicable provisions of Polish law shall apply.
These terms and conditions come into effect on 1 September 2024 and remain in force until further notice.
